Sponsorship Program Submission Agreement

This Sponsorship Program Submission Agreement ("Agreement") is made and entered into by and between Perfect Sports® ("Company") and the undersigned athlete or influencer ("Participant"), effective as of the date of submission.

Purpose: Participant is submitting personal information, media, and other related content ("Submission") to Perfect Sports® for the purpose of applying to the Company's Sponsorship Program. This Submission will be used to assess eligibility for the program and, upon acceptance, to create a public profile on Perfect Sports®'s website and for marketing purposes through the Company's influencer management platform.

Consent to Use Information: Participant hereby grants Perfect Sports® the right to use all information and media included in the Submission for the purpose of evaluating the Sponsorship Program application and, if accepted, for creating and maintaining a public profile, and for marketing and promotional activities related to the Sponsorship Program.

Accuracy of Information: Participant confirms that all information provided in the Submission is accurate, up-to-date, and does not infringe upon the rights of any third parties.

Ownership and Rights: Participant retains ownership of the media provided but grants Perfect Sports® a non-exclusive, worldwide, perpetual, and royalty-free license to use, reproduce, distribute, display, and perform the media in connection with the Sponsorship Program and related marketing activities.
